Basic ID Verification Form


All applicants will be required to have at least 3 x original ID documents (see list of Acceptable ID) to be checked by a nominated ID Checker in accordance with the Basic Application ID Checking Guidelines.  

To have your ID Verified, please download and print our ID Verification form by clicking on the button below, and arrange for your completed ID verification form to be signed by your nominated ID Checker

Please note:

The nominated ID Checker must be a professional person or someone well-respected in your community (‘of good standing’) like a:

  • bank or building society official
  • councillor
  • minister of religion
  • dentist
  • chartered accountant
  • solicitor or notary
  • teacher or lecturer

The nominated ID checker shouldn’t be:

  • related to the applicant (by birth or marriage)
  • living at the same address as the applicant
  • in a relationship with the applicant


Incorrectly filled-in applications, queries, or incorrect ID will cause delays and may result in the rejection of your application.
In such cases of rejection, or late cancellation, administration charges may be applicable.
